Online registration as a patient in our practice is easy and fast. Choose the correct form for you and make sure you keep your health insurance pass available: Ofcourse, you can also register yourself or your child by visiting our practice. New patients can be registered on working days between 8.00 - 12.00 hrs and between 14.00 - 16.00 hrs. If you prefer to visit us, click here to print out a registration form. You have to fill out this form and bring it with you when you visit our practice for registration. |
